The Guided Care Model

Description
The Guided Care Model was created in 2001 by researchers at John Hopkins University. The model was created with hopes to improve health outcomes and reduce spending for aging Americans with multiple chronic conditions by better managing care. In Guided Care, uniquely trained nurses establish care plans, educate and support patients and their caregivers, and coordinate care among providers.

Results / Accomplishments
The Guided Care model was tested in a randomized control trial with over 900 patients in the Baltimore- Washington D.C. area at eight primary care facilities. Reports include improved quality of care and a reduction in home health usage.However, the model did not seem to reduce the use of other high-cost services.
Nevertheless, a subset of the patients in the trial who were insured by and received primary care through Kaiser Permanente had significant reductions in admissions (47.2%) to skilled nursing facilities and days spent there (51.6%).
